Scope-creep flags
Each item is labeled as either a fix (part of the contracted work) or a new request. This makes scope creep visible: if a 'small tweak' is actually a new ask, it's flagged, giving you a clean record for billing conversations.
Confidence
Low-confidence items — where the client was vague — are marked so you know which notes might need a quick clarification before you spend time on them.
